apps/ely-doc/src/navigate.rs:139 — The current outline item compares only vertical coordinates, so headings in adjacent columns are attributed to the last column even when the cursor is in the first. Reproducer with the real macOS worker: use the navigation LONG fixture with column breaks instead of page breaks, apply page.columns with 2, and jump to the first heading. The heading rectangles are (1418, 1418, 4985, 653) and (6404, 1418, 4985, 275); the cursor lands at (1418, 1658, 0, 414), but current returns Some(1) instead of Some(0). Two isolated runs fail. A real window independently shows the caret and AX paragraph at “First part” while the outline marker and emphasis remain at “Second part”, contradicting C08.6's cursor-following current item. Fix direction: determine the current heading using document order and its position in the page/column, accounting for the cursor's horizontal position; add a two-column regression that checks the rendered current item after jumping between columns.
